Registration

Registration via Internet:

1. Check if the business name is still available.

2. Reserve the business name.

3. Download Registration Form.

4.Print downloaded form.

5. Bring the following to DTI:

• Completed BTRCP Form No. 17A

• Two 2X2 color ID pictures of applicant which were taken within one (1) one year immediately preceding the filing of such application. The pictures must be identical, clear and signed at the back by the applicant.

• Filipinos whose name are suggestive of alien nationality must submit proof of citizenship such as birth certificate, PRC ID, voter's ID, passport

• if the applicant has a foreign sounding name, acquired Filipino citizenship by naturalization, election or by other means provided by law, he must submit proof of his Filipino citizenship such as:

  • Naturalization certificate and Oath of allegiance,

  • Affidavit of election or ID card issued by the Bureau of Immigration and Deportation, or

  • Valid ID card issued by the Integrated Bar of the Philippines (IBP) or Professional Regulatory Commission (PRC)

• Processing fee of P300.00 + Documentary Stamps as required

• If place of business is owned, photocopy of: Official Receipt of Real Property tax for the current year and Transfer / Original Certificate of Title (TCT / OCT)

General Requirements:

• Duly notarized Standard Petition From

• 1 x 1 Photo of Applicant

• For Corporations and Partnerships:  Photocopies of; SEC Registration and Articles of Incorporation, DTI Registration

• For Single Proprietorship: Photocopy of DTI Registration

• Mayor's Permit

• Itemized fixed assets and operating capital duly notarized

• Floor plan of working area with dimensions in meters

• Plant address, location plan, and site plan

• Notarized Contract of Lease if site is not under applicant's name

• List of products to be manufactured / repacked

• List of personnel and their scholastic attainment

• Duly notarized Licensee Agreement for Licensee

• Duly notarized Franchisee Agreement for Franchisee

Manufacturers

Repackers

• Notarized Agreement from Manufacturer with a stipulation that both Manufacturer and Repacker are jointly responsible for the Quality of the Product.

• Photocopy of the License to Operate of the Manufacturer

• Floor plan of working area with dimensions in meters

Importer of Raw Materials / Finished Products in Bulk

• Foreign Agency Agreement duly authenticated by the Territorial Philippine Consulate

• Certificate of Status of Manufacturer issued by the Government Health Agency in the Country of Origin duly authenticated by the Territorial Philippine Consulate

• Brand name/s and packing size(s) / Country of Origin of Products

Exporter (Manufacturer / Repacker)

• Agreement from Supplier / Manufacturer

• Photocopy of License to Operate of Supplier / Manufacturer

• Name and Address of Consignee (if available)

Technical Requirements:

• For Corporations: Organizational Chart indicating qualifications of key personnel in Production and Quality Control

• List of Production Equipment including specifications

• List of Quality Control facilities and equipment (if any)

• Flowchart of manufacturing processes with emphasis on identification of Critical Control Points

• Detailed description of manufacturing procedures

• Quality Control procedures enforced in the plant

• Certification of current laboratory analysis from recognized laboratory

• Water source

  • Municipal Health / Sanitary Engineer approval for plants outside Metro Manila

  • Utility Receipt for plants within Metro Manila

  • Municipal Health / Sanitary Engineer approval for Deepwells

Requirements:

• Notarized Letter of Application and Application Form

• Product Information:

  • List of ingredients in decreasing order of proportion. For additives with prescribed limit, the amount must be indicated.

  • Certificate from flavor supplier that the flavor components are recognized safe and suitable for human consumption either by the US Flavor and Extract Manufacturers Association (FEMA) or the US Food and Drug Administration (USDA)

  • Finished product specification: Physico-Chemical and Microbiological

• One sample of the product in commercial presentation. For initial / new product application, a proposed artwork of the label may be submitted.

• Labels and labeling materials used in the product (to be attached to documents).

• Certificate of analysis of the finished product. Include the analytical method used. Submit tests results, which are critical to the product. Analysis may be conducted by BFAD's recognized laboratories.
